Forms to Complete Before Your Session
Once you have called and scheduled your first appointment, you will be sent an invitation the the Hipaa secure client portal where all the paperwork can be filled out securely online. If you prefer to use a paper copy, please print, complete and bring the "Client Information and Office Policies" with you to the first appointment. The Privacy practices document is for your information only and does not need to be printed.
